A young woman brightly waves over to you and addresses you by name.

No, not your character's name, your name.

The entity known as the Outside Woman is an enigma; while plenty of creatures have seen her, no one knows exactly who she is or where she comes from. While she is otherwise rather forthcoming with information she has picked up elsewhere, she seems incapable of divulging any information about herself - and considering Divination attempts inevitably return "Right behind you" when used to ascertain information about her whereabouts, it is generally considered inadvisable to snoop.

Not so much because she herself minds, more because it tends to end up with her being present, regardless of any wards or magic used to keep her away.

Combat
The Outside Woman is generally non-violent; however, if pushed, she will use World-Breaker's Hand with vicious efficiency.

Conflating Actors With Their Roles (Ex): Rather than perceiving the intelligent creatures around herself as individuals with their own hopes, dreams, and fears, the Outside Woman directly perceives their player instead. While she is vaguely aware of the distinction between the entities (at least, just enough to separate creatures by their "roles" in the world), she is incapable of using any name except the name generally used by members of the player group. As most creatures are considered to be "played" by the DM, she has a marked tendency to refer to most creatures she encounters by the DM's name.

Divine Rank 1: The Woman Outside receives the following benefits for her Divine Rank; certain benefits are subsumed or replaced by her other unique features. Her Portfolio is over Transgression - from those seeking forbidden knowledge to heretics to actions against her inviolate person, she watches with empty, mindless, smiling eyes.

• A +1 Natural Armor bonus to AC, as well as a +12 Deflection bonus to AC.
• A +1 bonus to all d20 rolls; in addition, The Woman Outside does not automatically fail attack rolls or saving throws on a natural roll of 1.
• Immunity to polymorphing, petrification, or any other effect that would alter her form against her will.
• She is not subject to ability score penalties, ability burn, ability damage, or ability drain.
• She wonders why you enjoy murdering fake people.
• She is immune to electricity, cold, and acid, even if the attacker is a deity of higher divine rank. She is also immune to disease and poison, stunning, sleep, paralysis, and death effects, and disintegration.
• She is immune to death from natural causes, as well as death from massive damage.
• She is automatically aware of any Transgressive action that involves at least one-thousand people.
• She knows why you lie to yourself.
• Twice per round, she may automatically perform any action related to performing or punishing Transgression, as long as those actions have a DC of 15 or less.
• She may speak and understand any language, and may speak directly to any creature within 1 Mi.

Perfect Concept-Self: The Outside Woman is a woman roughly 24 years in age, with a normal human face, a full set of human limbs, and bright scarlet hair. She is kind to animals, friendly to others unless they have proven themselves unfriendly, and is generally charming. She speaks Common with a slight accent, is afraid of your dreams, and has replaced her organs with maggots. All of the above statements are true; any effect or spell that would make any of the above untrue automatically fails, regardless of how much power is put behind it.

In addition, the Outside Woman uses her Constitution modifier for Will saves in place of her Wisdom modifier, uses her Charisma modifier for Reflex saves in place of her Dexterity, and may act, converse, and plan intelligently despite the fact that she lacks an Intelligence score. She is considered to have DR 20/-, as well as Resistance 20 to all forms of damage (this replaces her DR 15/Epic and her Fire Resistance 6 from her Divine Rank).

Finally, the Outside Woman's Immediate Action does not consume her Swift action that round, or vice-versa. In addition, no effect short of death may prevent her from using her Immediate Action, or either of the automatic actions allowed by her Divine Rank. Unlike a normal creature with a Divine Rank, she does not possess a Divine Aura, cannot grant spells, cannot communicate remotely, has no Godly Realm, does not possess Spell Resistance, and cannot use Greater Teleport at-will.

Real But Unreal: Whenever the Outside Woman is not being directly observed, she no longer possesses a relative position within the world. Whenever an attempt to directly discover information about her is made (or she simply feels like re-entering reality), she may resume having a relative position in space and time.

While this does allow her to appear in close proximity with whatever creature attempted to learn about her (she enjoys manifesting right behind them), it is not a teleportation effects and is not subject to any effect that would prevent a creature from entering a plane. The only restriction on this ability is that she must appear in a position where the creature attempting to discover information can see her (or otherwise sense her with their primary sense).

Any Divination effect that would attempt to reveal information about the Outside Woman that is not listed under Perfect Concept-Self instead reveals one of the truths listed by that ability. Unlike other topics regarding herself, the Outside Woman is perfectly willing to share information about how she can breach any form of restriction with regards to instant travel. Her response tends to simply indicate that she did not, in fact, exist until she decided to return.

Unless she has truly pressing business, the Outside Woman will respect posted signs or politely worded requests for privacy. Most people simply don't ask.

World-Breaker's Hand (Ex): The Outside Woman's sole Salient Divine Ability, World-Breaker's Hand functions in exactly the same manner as Alter Reality, with the following exceptions:
• World-Breaker's Hand cannot be used to create or make permanent. It can only be used to diminish or destroy.
• The save DC for World-Breaker's Hand is 32, and any spells it mimics have a caster level of 20.
• World-Breaker's Hand may be used to counter spells, powers, or similar effects used against her; this still requires that she readies an action.
• Any of the effects of World-Breaker's Hand that solely target herself or a spell or effect affecting her may be performed as an Immediate action. She may choose to restrict an effect that would normally target an area or multiple creatures to solely target herself or a spell or effect affecting her.

Considering that it said that she replaced her insides with maggots, I think it's more like his ex.
