This is a concept archetype I created for a campaign that very heavily features demons (Not Out of the Abyss, but could work for it)

Barbarian Archetype: The Hellraiser
-3rd Level: Painbringer: While you are raging, as a bonus action, you may charge your weapon with fiendish energies. On the next successful weapon attack you make during your rage, you deal an additional 1D6 psychic damage.
-6th Level: Hellish Power: While you are raging, for every 10 points of damage you were dealt since the beginning of your last turn, you gain a cumulative +1 bonus on attack rolls this turn (ex. If you had taken 19 points of damage, you only have a +1 on attack rolls, but if you took 20 damage, you have a +2).
-10th Level: Lifedrinker: When you deal damage to an opponent, you may regain a number of hitpoints equal to your Strength Modifier (Minimum of one).
-14th Level: Painful Rage: When you start a rage, you may choose to make it a Painful Rage. While you are in a Painful Rage, anyone who deals damage to you takes 1D8 psychic damage. Damage dealt in this way does not trigger Lifedrinker.

Two things: One, why would you ever NOT do a painful rage?

Two, sack of rats. You can carry around a sack of rats, smack it around between combats, and heal up entirely.

Painful Rage should either be once/day or it should drain your HP (I mean, that would be perfect for demon-inspired power, but perhaps harder to balance). Then it becomes a choice with a cost.

And Lifedrinker should only work while raging. That almost solves the bag of rats problem. Or you could make it temporary HP. Or tie it to Painbringer and make that a Con mod times/day ability.
