Countercharm, and some other abilities, refers to creatures within X number of feet of the user. But how is that supposed to be regarded? Is it X feet as the character walks which includes X feet diagonal from the user or is it a sphere of radius X centered on the user?

Sphere of radius X centered on the user.
Edit -> However, if the character is walking while doing it, it would follow them, meaning that the effect would end where they started, and move along with them to end at a sphere of radius X at their ending location.

Characters don't walk X feet "diagonally" unless you're using a grid. And even then it's a simplified grid.
