If a creature happens to have both regeneration and fast healing in a round, do they stack to heal damage? (I.e a Troll with Lesser Vigor or a Planetar with the Saint template)

It depends.
A creature with Regeneration only takes lethal damage from certain specific sources as noted in its entry. All other damage it receives is treated as nonlethal damage.
According to the Rules Compendium, pg. 74, "A creature [with fast healing] that has taken both nonlethal and lethal damage heals the nonlethal damage first." So if the creature currently has only nonlethal damage, it would heal an amount equal to its fast healing plus its regeneration. If it only has lethal damage, it would heal an amount only equal to its fast healing.
However, if it currently has both types of damage, the nonlethal damage would be healed before any lethal damage is. I seem to recall a general rule that creatures may apply their own effects in whatever order is most beneficial to them. So it is likely okay to use regeneration first to recover nonlethal damage, and then heal what's left over with fast healing. The correct answer is, "Ask your DM".

I don't believe there is such a rule in this edition. At least, I don't remember seeing it, and I couldn't find it in the Rules Compendium.

Indeed this is not a rule in 3.5. It's merely a guideline suggested by the FAQ which, as we all know by now, isn't RAW.

"Ask your DM", I guess, is the correct answer to whether regen or fast healing goes first.

If you break a wall with a Colossal Adamantine Morning Star, do you break a 20ftx20ft square (The Colossal Adamantine Morning Star is considered a Gargantuan Object)? Or just a 5ftx5ft square? The d20srd only mentions hp per inch of thickness.

While normally you would choose to attack a 5ft square regardless of weapon size, you're not doing this. You're attacking an object that happens to span an area larger than 5ft square.

I can't seem to find anything that says you do. So no? Is there a particular wording that makes you think otherwise? Because there's a spell dedicated to detect scrying attempts (detect scrying) which implies the target is oblivious to scrying without it.

There isn't. I'm probably misremembering what I thought was a general rule for Scrying spells that allowed willing target to know who the caster is and purposefully fail their Will save.

How do Substitution levels and PrCs interact? Lets say I want a planar substitution level at Sorcerer13, but I'm taking a PrC at that level. So at Sor10/AbjurantChampion3 can i take the substitution level? Or do I have to get 3 more sorcerer levels? Or do I have to take a sorcerer level for my 13th level?

They only interact with the class they are substituting. If the planar substitution exchanges sorc 13, then you need 12 sorc levels.

Wait so you're telling me Sor10/AbjurantChampion3 has to take an additional 3 sorcerer levels to take the Sor13 substitution level? But the substitution level says you get this benefit instead of new 6th level spell known. How can a Sor10/AbjurantChampion3/Sor3 get the benefit instead of 6th level known when he doesn't get a 6th level known at ECL16?

Yes. Only a Sorcerer of lv 13 can take the Sub Level.
He can't. If you don't get a 6th level spell known, you can't take the sub level, since you can't give up the benefit.

Couldn't the character just retroactively lose whatever spell they learned at Sorcerer 10/Abjurant Champion 3?
The text does say, "From now on, the sorcerer’s number of 6th-level spells known at any class level is one less than the value shown on Table 3–17, page 54 of the Player’s Handbook."
That would seem to allow it.

The language of the Planar Sorcerer is "This benefit replaces the ability to learn a new 6th-level spell"
After he's already learnt the spell, he can't lose the ability to learn it.

- Skill Shards (MIC p.185) can give a +2 or +5 bonus to a single specific skill; however, they are consumable items.
- Seven Veils (MIC p.211): 3 pieces give a +2 competence bonus to Spellcraft checks.
- Spellsight Spectacles (MIC p.138) gives a +5 bonus on Spellcraft checks, but only to decipher scrolls.
- Tome of Ancient Lore (MIC p.189) +5 competence bonus to Spellcraft for 24 hours after perusing it for an hour. This is the best match I found.

The demilich template can be applied to any wizard, sorcerer, cleric of at least level 21 provided they are a lich. Would a cleric 17 who is a lich (so +4 LA) be considered 'level 21?
Edit I found the answer. It's page 209DMG. Answer is yes.

Potions can be made with any spell of 3rd level or lower.
Some options include Swim (SpC), Aquatic Escape (CS), Alter Self (PHB), Primal Hunter (Dragon Magic), Water Breathing (PHB), and Webfoot (SW)
There might be others, but these should suffice.

Forgotten Realms Campaign Setting has stats for both a Bandoleer and its Masterwork version on pages 95 and 96 (Mundane Items). They hold 8 and 12 items respectively.

Are the attack opportunity presented by moving out of a threatened space and the attack opportunity presented by moving into an opponent's space separate or the same? I.e., if a character has Combat Reflexes, can they make two attacks of opportunity on a swarm that moves out of a threatened space and into the character's occupied space?
