Oh, a two-part question! I know that one. This bearded man in Azure City prison is standing on a bucket in #458 before Nale breaks out. (Nale uses a better footstool than a bucket.) That man has class levels, and was committed for kidnapping, as we found out in #420, when Hinjo releases Tsukiko, Belkar, and that guy who wants to assassinate Hinjo.
Update: Personification answered faster, so I won't ask a question. Personification: does the word "no" still count if Haley speaks it ciphered, such in #393? It's unlikely to matter, he doesn't say "no" enough times for what you asked, just wondering.
Update: Personification: nice tough question by the way. It's not #694, that only has eight instances of "no" in two panels together. Nor is it Miko's “NO! No more compromises, no more delays, no more backing down before the battle is won!” in #406. And Belkar only exclaims a single big “NO!!” when he realizes that he'll never kill Windstriker. It's not #894 either: that's when Nale says “No no no no no no NO!” as he sees the message on the gate in Girard's pyramid.

8th panel of page 1 and 11 of page 2, 12th panel, 2nd, 7th, and 8th panels, 2nd, 3rd, 4th, 6th, 7th, 8th, 9th on the first page, 2nd, 3rd, 4th, 6th on the second. Don't know if any other purple pendants exist, but these are all the appearances of this one for good measure.

I think that's after the Escape scene, when nothing dangerous pops out to kill O-Chul after he taunts fate with his words. I know this one because the Escape is one of my favourite story arcs, and I've re-read it many times, often to clarify the clues we have about the MitD. You might mean something else, but Elan saying “dun dun dun” when Trigak died in #21 doesn't seem to fit because I expect Trigak's dramatic return later, not right then.
