Haha, well, to be entirely honest, I modified them from another source in turn. The combat tracker as it stands is essentially heavily modified from an earlier incarnation one of my prior DMs used.
I'm glad to see it getting popularized on these forums, because it really is a handy tool that is a huge help in facilitating PBP, and its use can only increase the quality of those games.
