Dragon Age 2. The woman is the female version of the protagonist who rises from nothing to become the most powerful noble in the city. The guy in the back is the Arishok, one of the top level leaders of Buddhist-Communist Ottoman Empire who terrifies everyone in the city by staying in the harbor with his soldiers "waiting for a ship home", for three years.

Actually, it was invented by the French nobles and royalty just before the French Revolution, where it saw the most use in executing the class that invented it. Also, it was invented as a more merciful manner of execution than hanging, as it was a swift death with no chance of suffocation.
