My little slice of WoD Pie, VTM: Cthonic Wars
I've recently had a go at a system conversion, using the OWoD setting and the NWoD rules I've created some sort of Frankenstein's Monster of a game system.
I wanted to use the rules from NWoD 2.0 to be exact, it seemed rather neat and removed a lot of the tacked-on bits and pieces from it's previous incarnation, but I felt quite unhappy with the handling of the clans in Requiem and it left me with a bad taste in my mouth, and the rules for the Old World had me feeling very much the same after playing about 2 years in a Changeling: the Lost LARP (We used Mobile phone dice-rollers).
Then I wanted to add my own little twist to the whole "secret entit(y/ies) behind the scenes". I liked the whole idea of the God Machine, but felt that it needed to stay where it is in the NWoD setting. I set my sites on the writings of H.P. Lovecraft instead.
I would have Kindred Fighting creatures from beyond human understanding. I would have them struggle against creatures of Yog-Sothoth, Cthulhu, Nyarlethotep and perhaps struggle with the fact that Kindred too were not of this little island on the sea of infinity.
Naturally this all replaces the otherwise ham-fisted and awkward Abrahamic-infused lore of the usual Masquerade game.
What are your thoughts playgrounders?

Are the vampires in any way related to the Mythos creatures? If so how? Are they descended from those who had illicit relations with monsters? Were they an ancient cult that was blessed/cursed by it's patron and then abandoned? Perhaps they some form of mythos parasite that inhabits the bodies of the dead?
Why are the vampires fighting the Mythos creatures instead of their usual response to other supernatural beings (try not to look directly at it until it goes away)?
Is the Camarilla/Sabbat/Anarchs/misc smaller factions war still a thing? Do they perceive the mythos creatures differently? Do they consider the mythos creatures bigger threats than the other vampire factions?Rural Reign An Original Superhero Webcomic Written by Me and AteMozzarlla

One of the few things I like about the Masquerade setting was the story of the supposed "Caine and Able", in my twisted version of the history they are both Cultists to Yog-Sothoth. "Able" in this divergence was the one who suggested to "Caine" that he sacrifice him to the creatures from beyond. After the deed is done "Caine" has a moment of clarity (an "Awakening" if you will) and runs from the rest of civilization but by knowing too much of the One-in-All he has already been cursed 3-fold (You know the drill: weakness to fire, vulnerability to sunlight and the Beast Within that hungers for blood).
Aside from where it all began the world is still very much the same, at least on the surface. Kindred have been kept in close-to as much ignorance as possible with a few finding out the unfortunate truth of their existence, many being driven mad by it.
The Camarilla thrive in ignorance.
The Sabbat only have small little bits and pieces of the "Book of Nod" to go on, and even then a lot of the writings have been messed around with by those with an agenda.
The Anarchs as they are today are still a faction of personal freedoms and meritocracy.
People have become more aware in the past century and a half, and some Kindred have started chronicling the strange happenings around the world that seem to defy logic; conflicting 1st-hand stories about the actual events of the sinking of the Titanic, Wolfenstein and Hellboy-esque occurences of Nazi Scientists conducting odd experiments, disappearances at sea in the Bermuda triangle, strange radio signals hailing from an unknown source, reports of strange monsters.
