I am curious to understand how people think she might have died without reaching old age.

I believe he can only manifest in the vicinity of the Greenhilt Sword (plus some other restrictions, but that's the key one keeping him from visiting Julia)

Without giving SOD spoilers, the implication there is that he can only talk to someone under the following specific circumstances:

1. He's talking to his heir, that being the eldest living child, aka Roy, though it presumably did pass to Julia when Roy was dead. Alas for Eugene, however-
2. Said heir must have the family sword, intact, which had never applied to Julia.
3. Also apparently, everyone around the heir must be asleep, which is tougher in a boarding school environment.

And that's only the rules we know about!

Also, apparently the school has pretty good wards against that sort of thing.

Because the rules say he can't*. He can only visit Roy while he is holding the Greenhilt Sword and no one else is around. Or awake. Or something.
At least that is how I understood the situation.

*As in is unable to, not isn't allowed to
