Ways to get 4 arms as Human?

So I'm looking to ways in 3.5 to get 4 arms. I've seen a lot of ways of doing it by being the Thri-Keen race or however you do it. But I'd like to do it as human, or gnome, or a base race.

I know you can permanency one of the spells that adds two extra arms with DM permission or an item of it. Are there any other ways you can make this happen?

I'm not sure if a graft would allow you to add extra arms, but there's certainly arm grafts, so it might be worth talking about with a DM at the very least
