My take on the issue: it should not by itself be too horrifying. However, it should make it clear that stuff in here is horrifying. I found Caracol's picture to be ideal since it was a great teaser for the horror to come.

Use the fire breath of a roach in a stream arcing out from the nude Redcloak's waist in a censoring manner as O'Chul is strapped to a wall in chains, belly to the wall. The arc of fire hits the wall between O'Chul's knees, and Redcloak as a sadistic grin on his face.

We'd have to one-up the time one of the roaches had its sensitive little fire-bladder squeezed by Miko.

How about Miko, as one of her trials to earn redemption, is polymorphed into a giant roach (work in Kafka references if you feel like sounding smart. A rotting apple lodged in the back is disgusting enough to qualify for horror) and is viciously and viscously pawed by giant clones of herself.
