While the charm of the Taco Fairy cannot be denied, my heart (and vote) were stolen away by the fell clutches of Bloody Handed Jack.
Ingredients
2oz Djinn
5oz Water
1 Lime Wedge
Instructions
Pour Djinn and tonic water into a glass filled with ice cubes. Stir well. Garnish with lime wedge. Serve.

I'm going to vote for the Whimsycap Toadstool. I like how it's relatively harmless but has endless potential for traps.

I have to vote for the Morrigna. My fey need a few epic figures.
Definite runner up would be The Woodsman. It isn't as other-worldly as the rest of the entries but I think it's potentially a very very useful monster. Extra points for having a clear literary source.

However, Bhu's marvelously malevolent monstrosity definitely comes in at second place.

So...many choices.
I like the toadstool. Cool, but a little bit dull when compared to other things. I dont think I will vote for the unique ones in this round because of the limited use you get out of them.
Cant decide yet. Will update with vote later....

I am voting for the Whimsycap Toadstool as well. Taco Faerie was delightful too and I have a fondness for the Woodsman.
DebbyP.E.A.C.H. Please Evaluate And Critique Honestly. Being nicer and kinder doesn't hurt either. Note I generally only critique 3.5 and Pathfinder material.
Please, please, please when using non-core material, cite to the books. There are too many books to wade through to find the one with the feat, special ability or spell you use.
