Cheyanne has the class feature “Forewarned,” which allows her to “always act during surprise rounds.” I figure that means she acts last in the Surprise Round. I’ll check the full text later when I can.

A nornkith can always act in the surprise round, even if he does not notice her enemies, though she remains flat-footed until she takes an action. In addition, she gains a bonus on initiative rolls equal to half her monk level (minimum +1).

Manifesting powers follows the same rules as casting spells to determine attacks of opportunity.
Injury: Getting hurt or being affected by hostile psionics while trying to manifest a power can break your concentration and ruin a power. If you take damage while trying to manifest a power, you must make a concentration check (DC 10 + points of damage taken + the level of the power you’re manifesting). The interrupting event strikes during manifestation if it occurs between when you start and when you complete manifesting a power (for a power with a manifesting time of 1 round or longer) or if it comes in response to your manifesting the power (such as an attack of opportunity provoked by the manifesting of the power or a contingent attack from a readied action).
To manifest a power while grappling or pinned, you must make a concentration check (DC 10 + the grappler’s CMB + the level of the power you’re casting) or lose the power.
I don't think this is working out...Monks have a reputation for being ineffective, but it's my dice that are causing all of Cheyanne's attacks to miss...literally every attack so far this adventure!

10+4+1 = 15 , Millie is treated as if she rolled a 15 and this should pass
I do not need focus to manifest the armor and so that should also take effect

It's usual swinginess of low-level Pathfinder combined with the terrain (we can't really flank in these swamps).

There were 2 crocs adjacent to you when you started to manifest, so one took a swing. Luck of the dice. Lucky the crocs are not interested in sharing, they each are only targeting 1 pc.
But if it makes it any better, the next combat doesn't focus on reach, grapple, swallow hole, or large vs small
