Etho's design is borked (or perhaps it requires a complete shaft) in that the pearl routinely impacts on the underside of the lilypad, leaving a potential user either falling (if they weren't expecting it) or trapped in the source block (if they were).

Not sure how you are having trouble. works perfect for me, out in the sky. The enderball is intended to hit the lily pad and you swim up through it and either step out or keep going up.

Throw the enderball striaght up at the lily pad,
hold down space so you are swimming when you teleport into the water.
Repeat as necessary.
