To answer the initial questions asked:
The game is DnD.
Encounter:
5 level 5 characters, fighting a zombie 5-headed hydra and about 15 zombie commoners. hydra on one end, zombies on other end of a 20 ft wide, 200 ft long hallway. Killed wizard with hydra, got barbarian into negatives, and brought another player to exactly 0 hp.
The problem was that the group tried to split up and fight both at once, instead of taking out one group or the other. A wizard and a barbarian fighting alone versus the hydra allowed their death/near-death.

First off, is this EL to high for the party or was the party just being stupid?
Also, I should note that the hydra killed the wizard on three solid hits and one critical, bringing him from full health to death.

As for the other multiple pages, I haven't looked at everything yet.

hi OP

welcome to the cult of snuggles

But i would personally say that scenario might be a BIT over the top-EL wise. But they had choices, and made a poor choice by splitting up. They should have mopped up the zombies, and then pegged if they didnt have the reserves left over to take out the hydra
