Since all Shinigami seem to start their life as a gifted plus (or, as in Ichigo's case, Human Fullbringer Quincy) there must be some point, when the creation/summoning of a Zanpakuto takes place. Maybe the swords are removed parts of the soul, subconsciously shaped into a new form by the soul's owner. It might be a very fast process, similar to Rukia turning Ichigo into a Shinigami.

Well, a different way would be Human (for the extra feat and skillpoints to put into swordplay, shunpo and mary-sue skills)
Race= Human, class is Mary-sue with the Shinigami, Quinchy, Fullbringer and Vizard templates added, with the Combat Brute ACF granting extra reiatsu in exchange for reiatsu control and kido abilites. Apart from the templates highly unoptimised...

Considering becoming a Shinigami involves sort of splitting off a part of your soul into an intelligent being in order to manifest your power, I'm not surprised it isn't natural. How much you want to bet that the original theory of Zanpakuto's came from observing how Hollows are born? Remember that Ogichi/Shirosaki/whatever you want to call him said he was essentially the same entity as Zangetsu. However Quincies work is probably related.

How do you know he's the 4th one they'll go to?

The way he presented himself he looked sort of like the unofficial leader of the group, so he'd most likely have invented the biggest thing and would thus likely be shown last.
