It happens when a game idles for too long.
It's actuallu one of the big reasons I don't run often, and why I like to run one player games: A single player has fewer delays.

Well, I will refrain in case you want to continue Rhy.
But I will give some metaknowledge, and some actually in world knowledge.
The planar cosmology is a bit different.
The elemental planes are coturminuous. Meaning they overlap, there are three other planes 'transitives' which are likewise. They are different from standard.

The Ethereal Plane (the dreamscape). When you meet someone you have never seen before there is a chance they are real. Strong enough dreams and nightmares actually manifest here.

The Astral Plane (The Soulscape). The spirit is very much in force here, and a beings true form cannot be hidden while even touching on this space. Magic is thought to originate here.

The Shadow Plane (The Fade). Very seldom is anything here seen as it truly is. Anything originating here, while tangible is never truly real. Even a sword which you bring back from this place. A Holy Avenger acquired here may seem to act like one, but any magic it has is undetectable, and any creature or object from this place merely ceases to exist when damaged or destroyed. Including Demons (Which are not always Chaotic Evil, but are never entirely real. The more powerful ones tend to be more cunning. Making deals with 'real' individuals in their quest to become real. Stealing the essence of others, or even objects in order to take on physical forms. As an illusion is never quite as potent. What a demon truly seeks is something is can possess but never have of its own, a soul.
