It's probably come up before, and perhaps it's too ambiguous of a case -- but might you not be missing O-Chul's first appearance, in strip 290?

That character has been declared not to be O-Chul. He's listed as "Fellow-Medium Sized Paladin", with two appearances.

Is the change in eye color worth noting?

I think we should make a clear decision if we are counting Malack as Lizardfolk or Malack as Vampire in the alterations list.

We don't keep track of Xykon as a lich, so no. Malack has been both lizardfolk and vampire for as long as we've seen him in the comic. If we get a flashback to a time when Malack was not a vampire, we will track "Malack as mortal".
