For smoke:

2 squares of smoke block LOS. This includes the final square you're looking in, but not the square you start from.

Thus, if I was in B12, I can see the whole board, because there's no more than 1 square of smoke between me and any square. However, most of the board gets concealment.

If you're in J12, you cannot see into B12, because you'd have to look through C12 and B12, and the two squares block LOS.
